Job Description

Looking for a Team Manager to join a team in Kingswood

37 Hours per week. Contract position.

This is an urgent booking to support the COVID-19 pandemic.

Full clean driver's licence and access to a car (or able to use another appropriate means of travel) to travel to a range of locations including home visits.


To carry out duties and responsibilities of Community Care & Housing Act 1970, NHS & Community Care & Housing Act 1990, Carers (Recognition and Services) Act 1995, Carers and Disabled Children's Act 2000 and to have an awareness of legislation relevant to working with vulnerable adults i.e. legislation in respect of Housing, Mental Health, Disability and Racial Discrimination, Human Rights, Asylum Seekers etc.

In partnership with vulnerable adults and their family carers, to co-ordinate and assess their health and social care needs, including a financial assessment, within the framework of the relevant legislation, Departmental eligibility criteria and matrix and vulnerable adults policies. To devise, record, cost and negotiate resources required and implement an appropriate care plan, involving all key stakeholders. To review and monitor the care plan and the quality of the service provision making changes in response to changing needs and re-negotiating service provision where appropriate.

To contribute to budget management by the strict costing of care plans, adherence to Net Cost Policy and applying via the Team Manager for exemption, where appropriate, or requesting additional funding in line with the Exception Special Needs Policy.

To promote anti discriminatory practice and the social model of disability in personal practice, within the team and with other professionals and colleagues both within and outside the Department.

To facilitate vulnerable adults to gain equal access to the relevant community services. This will range from raising awareness and skills within mainstream services to providing individual support to people.

To be responsible for action in situations where personal liberty or safety is at stake. To carry out risk assessments with service users, empowering them to reach their potential rather than restrict what they can do. To carry out duties under the Protection of Vulnerable Adults from Abuse Policy in order to protect service users from abuse.

To work as a member of a team, including multi-disciplinary team, and to build up good working relationships with other professionals within the Department and with external agencies in order to achieve the best outcomes for vulnerable adults and their family carers.
