Role Summary
We are offering an exciting opportunity for an experienced Advanced Nurse/Clinical Practitioner or Paramedic Prescriber to be part of a rewarding and dynamic service that is being delivered to patients in the South Fulham area. You will be providing exceptional care to patients referred to the Home Visiting service including initial history taking, clinical assessments, diagnosis and treatment. A company car will be provided.
Responsibilities
Carry out home visits for patients presenting a wide range of minor illness, minor injuries, and patients with long term conditions
You will be expected to see patients who are displaying symptoms of Covid-19
Acting autonomously and take responsibility for the assessment, treatment and be able to appropriately refer patients where necessary
Prescribing independently
Managing a clinician caseload, visiting patients in their own home in a timely manner
Work independently and collaborate with the wider team with GP support
Essential requirements
Passionate about our vision and incredible mission to combat health inequalities
HCPC or NMC Registered
MSC in Advanced Clinical Practice or equivalent (desirable)
Have completed Physical Assessment module or Level 7 in Minor illness and Injuries
Experience working in either a GP Practice/OOH/Urgent Care setting
Excellent communication & IT skills
